Request A New Faculty And Staff Network Account

NOTE: Printing and/or faxing this form is not required.

How to use this form:
  1. Read the instructions before using this form.
  2. This form is used to request the creation of a UCA Faculty and Staff account.
  3. The information on this form is necessary in order for us to be able to create an account. Fill in the form and click the “Send Request “ button at the bottom.
  4. The Contact Person you specify below will be notified regarding the account status.

Account Creation Rules

Enter the account name you prefer up to 15 characters. This will be used for network and Banner access as well as for your email address. Granting of requested account name is based on availability, format, and technical restrictions. Please do not enter "@uca.edu".

Account Names will generally be assigned according to the following format if the name is available: First initial followed by last name or First name followed by last initial. Accounts will not be named with only the first or last name of an individual.

Note, that account name changes are not trivial. If you do not specify a requested account name, do not call later indicating that you want something different than was assigned. Along the same lines, if you do indicate a name, make sure it is the one wanted

Individual account names will reflect the actual name of the individual, though common shorted versions are allowed (i.e. Bob for Robert, Debbie for Deborah, etc). Departmental names (email aliases) will reflect the name and/or function of the department. Generic names may not be used. For example, departmentchair@uca.edu would not be an appropriate selection.

Pool Drives

Pool drives or departmental shared folders are provided so individuals can share information and data among the department. Pool drives are compatible with both Windows and Apple computers. Each pool drive is owned by the department head/chair and requires his/her approval for individual access. Pool drives are backed up by the Information Technology Department each night to ensure data accessibility in case files are deleted. Below are the guidelines to request access:

  • Request access for new employees: Select "yes" at the Access to Pool directory required section of the new network account request form. Individual's will be given access to the requesting department's pool drive only.

  • Request access for current employees: An email from the department chair/head sent to UCA Helpdesk is all that is needed.
