How to use this form:
- Read the instructions before using this form.
- Fill in the information in the text boxes below then click on the "Submit Request" button at the bottom of the form.
- The information included on this form will be used in the creation of the departmental mailbox and inclusion in the UCA email address book.
- Incomplete forms will not be processed.

Account Creation Rules
Enter the name you prefer for your email address. Account Names will generally be assigned according to the following format if the name is available: First initial followed by last name or First name followed by last initial. Accounts will not be named with only the first or last name of an individual.
Granting of requested account name is based on availability, format, and technical restrictions. Account names are limited to 15 characters.
Note, that if you do not specify a requested account name, do not call later indicating that you want something different than was assigned. Account name changes are not trivial.
Individual account names will reflect the actual name of the individual. Departmental names (email aliases) will reflect the name and/or function of the department. Generic names may not be used. For example, departmentchair@uca.edu would not be an appropriate selection.
